  • Complete a lifestyle health self-assessment and review with a nurse to gain insights on the impacts of your current lifestyle on your overall health and wellbeing.  You will complete an assessment online ahead of the scheduled session and review the results together during your session.  You will finish the session with understanding of your current status in the pillars of health and wellness as defined by the American College of Lifestyle Medicine, along with resources to address areas of opportunity for improvement.

    This session can be repeated monthly or as needed for ongoing support and monitoring of health changes. Minimum suggested frequency for monitoring trending of lifestyle health factors is every 3-6 months*.

    Fee: $80**

    *6-month lifestyle health coaching package is an optional add-on service available for booking after the first session.  Inquire for pricing.

  • Lifestyle Health Circle 3-month Program

    What is a Lifestyle Health Circle?

    A Lifestyle Health Circle is a social support group that forms in order to provide connection with others who are integrating lifestyle health and wellness habits, related to the the 6 pillars of lifestyle medicine, including physical activity, nutrition, sleep, stress resilience, social connection, and reduction of substance use, into their lives.

    What is the purpose of this program?  

    The purpose of Lifestyle Health Circles is to provide social connection within a small group of 3-5 individuals facilitated by lifestyle health nurse; social connection enhances wellbeing and increases likelihood of healthy lifestyle habit change and maintenance.

    Who should be in my Lifestyle Health Circle?

    This is totally up to you! You might choose to circle up with friends, family, or be assigned to a group with others you haven't met yet. The choice is yours!

    Example Types of Lifestyle Health Circles: 

    Similar life stage

    Similar health issues (chronic illness, etc.)

    Similar interests

    Similar vision/ purpose/ goals

    Family/ housemates

    Cross-generational

    Lifestyle Health Circle Agreements: 

    • Bring an open mind and nonjudgmental attitude

    • Respect privacy (what is said here, stays here)

    • Each circle will then have the opportunity to create their own agreements to add to the foundational ones above.

    Process:

    1. Sign up for Lifestyle Circle package

    2. Complete Individual Lifestyle Health Self-Assessments on digital platform

    3. Meet as a group in location of your choice to review and co-create circle agreements

    4. Set individual goals and group goals

    5. Weekly circle discussion prompts in chat (Reflect & Reset Sundays)

    6. Meet monthly for 3 months for ongoing checkpoints on goals and social support

    7. Reassess lifestyle health status and plan at 3-6 month mark

    Fee:

    $200 program fee includes access to digital health coaching platform, lifestyle health assessment, guided weekly group chat discussion, and three monthly guided group sessions with circle members and lifestyle health nurse.

  • All workshops designed to be delivered in 1 hour, for up to 20 participants, at $150 per hour.  Any of the workshops can be customized and tailored to meet your organizational needs and goals.  Reach out to Libby for custom curation of a workshop topic just for you and your group!

    Lifestyle Health Assessment & Goal-Setting

    Receive guidance through an assessment of your current state of lifestyle health. Spend this time in a guided small group discussion gaining clarity on your personal state of whole being wellness and walk away with an actionable plan to level up your lifestyle health & wellness!

    Cultivating an Attitude of Gratitude

    Creating a habit of expressing gratitude daily supports a more positive mindset. In this session, learn about the science behind cultivating an attitude of gratitude for better health and well-being. Then participate by creating your own gratitude tree. 

    Guided Nature Walk & Shinrin Yoku

    Shinrin Yoku is an ancient Japanese wellness practice. Literally translated, it means ‘forest bathing.’ Being in nature is good for our well- being: body, mind, and soul. In this session, learn about the history of this practice, the science behind nature’s healing benefits, and be guided on a mindful nature walk on your campus or public trail.

    Full Plate Living Information Session

    Learn how the food we eat has a big impact on our bodies’ ability to fight illness, stay healthy, feel better, & live longer.  Sponsored by the nonprofit Ardmore Institute of Health.

    Mindful Meetings

    Learn through guided experience how to bring elements of mindfulness to your business meeting structure for effective communication and teamwork!  Lead and participate in groups and teams with intentional calm and a strong foundation.

    Book Club: Guided Personal Development

    Choose a health/wellness/personal development book for your group to read and then come together for a guided discussion. Libby has a list of book options or bring your own idea.  Great for team-building!  

  • Bring your lunch and join me on Zoom the first Monday of each month for Lifestyle Health Lunch & Learn.  In the field of lifestyle medicine, YOU are the primary prescription.  You hold the power to create a foundation of wellness through your lifestyle and behaviors.  I am here to support you in creating that foundation of wellness for yourself and your family!

    Each month, I will present information and education on a different topic related to lifestyle medicine and then open it up for questions and discussion.  Join the Zoom call to learn, discuss, and connect with others who are also on the path of healthy living.
